Tap Height, Weight, Blood Pressure, Body Temperature listed under the Add Observations and Body Readings header.
Set the Date and Time at the top.
Use the rest of the form to complete the Observations gathered at the time of the encounter.
The measuring systems for height, weight, and temperature are configurable in the Settings area of the app.
